What are the Requirements:
For Aliens/Immigrants:
- Fully accomplished Medical Examination Forms
- Three (3) 2×2 I. D. Pictures
- Original and two (2) photocopies of the required Laboratory and chest X-ray results
- Chest X-ray film/CD
- Certificate of Vaccination (if applicable)
- Original Passport
** Laboratory Tests and Vaccination Requirements may vary depending on the current WHO Guidelines
For Stewards, Food Handlers and In-Flight Catering Service Employees:
- Fully accomplished Medical Examination Form
- Two (2) 1×1 I.D. Pictures
- Original and photocopy of the required Laboratory and Chest X-ray results (including x- ray film/CD)

Issue medical clearance by giving two copies of Medical Examination Forms with Certified True Copies of Requirements submitted and by stamping passport with “BOQ passed” with date issued and receipt no. indicated
For Alien/Immigrant classified under Class A or B: Issue “Class A or B Certificate”
For In-Flight Catering Service Employees: Issue Health Card |

PROCEDURE IN FILING COMPLAINT
- Write or email us or fill up the Client Satisfaction Survey Form. Please indicate the following:
- Your complete name and signature
- Your contact numbers
- Date and Time of incidence
- The specific concern
- File complaint through any of the following:
- Drop Survey Form in the designated feedback/complaint box.
- Submit a letter or e-mail to Chief of The Medical Services Division
- Submit a letter to the Office of the Director: 25th & A. C Delgado Street, Port Area, Manila
- Both parties will be given a chance to explain within 24 hours upon receipt of complaint.
- A letter indicating the actions taken by the Bureau of Quarantine will be sent to the complainant through email, fax or mail
- Should the incidence require a preliminary hearing, both parties are required to attend the appointed date.
