 Who may avail the Service:

  1. Those who lost their International Certificate of Vaccination
  2. Those who want to replace their valid International Certificate of Vaccination

 What are the Basic Requirements:

  1. Duly accomplished Duplicate/Change Card Form
  2. For change card, valid International Certificate of Vaccination(ICV) or photocopy
  3. For lost card, indicate in the date of vaccination in the form for verification in the records
  4. One (1) valid Identification (ID) Card

(Note: Valid ID contains applicant’s clear picture, date of birth, signature, and signature of the authorized head of the issuing agency such as Driver’s License, SSS ID, GSIS ID, Philhealth ID, current Company/Office ID, Voter’s ID, Valid Passport or Police Clearance, Seaman’s Book. Any ID not included in the list shall NOT be accepted.)

  1. Authorization Letter from the ICV holder or the company signed by holder or authorized company representative

(Note: Authorization Letter should have the company letter head and must indicate the name of the authorized personnel and the name of the person vaccinated)

  1. Authorized representative should present a valid Identification (ID) card
  2. Vaccination Fee: Php 300.00

Note: The 8-minute processing time is for one client being served at one time. The time is extended when there are two or more clients.

            For those clients whose records are still in the logbook, they must find their name and date of vaccination at the logbook and present it to the screening nurse for verification.
